Supported Living Trainer IV- Jonesboro POS# 137 JA

Individual must be dependable and reliable.  Minimum two year experience working with disabled adults, minimum one year experience in supervisory role or bachelor's degree preferred.  Employee will work with disabled adults one on one.  Due to staffing needs, individual may be required to work with more than one consumer during shift.  Must stay awake at all times during shift and must stay with consumer at all times during shift, for the health and safety of the consumer.  Employee must follow all policies and be able to take direction from supervisor.  Janitorial work is required.  Must be able to sign and perform Supported Living Trainer I's duties list.  High school diploma or GED preferred and transportation required.

 

HOURS:

Monday - Friday - 7am - 3pm

Job Requirements

Duties:

Be awake and available to handle emergencies during the night

Assist  in orientation of new consumers

Bring food items/cleaning supplies, ect. from the pantry

Unpack and store food items/cleaning supplies, ect. in pantry on the days they are delivered

Nightly sweep of lobby, kitchen, dining room and bathrooms

Follow prescribed activites/methods to meet individual objectives for the comsumers plan

Help formulate new plans for treatment/training activities when indicated by changes in teh consumer's program

Make appropriate narratives and accurate objective reports

Related consumer progress to supervisor

Complete required training for job retention

Possess or be able to pass First Aid and CPR training

Possess a valid AR drivier's license and proof of automobile insurance

Help to meet applicable standards of sponsoring agencies

File daily case notes, monthly reports, ect.

Document nightly case notes

Respect the confidential nature of ALL consumer information

Request assistance from professional staff as needed

Promote a nurturing enviorment

Other duties and training as required.
